Quiz help w/database results

Hello all, I have developed a quiz on my web site, http://www.dreamsonthefly.com/templatemaster.html

I have accomplished this with the awesome help with one of your members.

I now need to be able to obtain the results with the test takers name, email addy, other pertinent info, and the results in a data base so I can give away their prize.

Any help would be appreciated.

though I don't know what DB you are using but you must know how to access the tables and thus their columns using a server side language. So just write a results page or something that will give the results and who has the highest score. Maybe Im just not understanding how you want to find out the info.
