Mp4 file won't play

Mp4 file I have placed on this test page plays on test browser on computer but once uploaded has a (Q?) Quicktime symbol?
Why does it not play on the web?

I actually don't see a problem. The mp4 file works fine. Did you not put the right ID for the media player? Make sure that each player plays the specified extension otherwise it will not work.
