View Full Version : Stuck on creating a custom form

Im really kinda clueless when it comes to making forms, Ive always relied on just going to jotform and making a pre-made one, but it doesnt work for this problem.

I need to make a form, that sends an email to the shop with the details of a appointment request, the hard part is I need to make it look exactly like this..

so far, when i try to place it within my divs, everything shifts odd, and the sizing is off, and yeah... i really have no clue how to do it and am getting screamed at daily since it hasnt been done :/...help?

You need to position it absolutely within #main_image. Does that help?

I did, but thats not really my main problem, the main problem is i dont know how the heck to make the form look like that, I tried getting a base from jot form and editing it, but yeah, soooo didnt work as intended. looked ugly.

Have a look at these two resources on forms:

The A List Apart article is pretty old now but still has some useful things to say.
You're going to need a submit button as well, although one isn't shown on your design doc...

Have a look at them and have a try at producing the form, and post back if you get stuck.

Ok, so that worked awesome so far ( the 2nd one, haha i tried the 1st a lil while ago and just went " huh?") my only question now, is I know how to make the form submit where it pops up an email compose message thing in your default email program, but how do i just automatically make it send the email to the intended recipient with all the details located in the form?

You need to use a server side language to do this - have a look at this simple php example (http://email.about.com/od/emailprogrammingtips/qt/How_to_Send_Email_from_a_PHP_Script.htm).

Hi,

I think you should use some online softwares to create a form as per your requirements or if you are agree for spending some money there are lots of sites which you can use to make the form like Aweber, constantcontact,etc.
