would like to take some paper forms that a relative of mine has too fill out, and make a digitial copy of them, to be filled out not only from a print out, but on a PC as well.

The forms will look, and be very similar in function to things like this:-

The forms would need to have fixed text, text boxes, tick boxes, mulit-choice drop down boxes and other things like that.

What would be the easiest way of making something like this? the 1st thing that comes too mind is making it for a browser in HTML/XHTML/CSS or something like that. Or should i be looking at something like publisher, or a acrobat?

Any ideas?

I used to use this program called OmniForm at one of my previous jobs that would let you scan in forms and it would convert them into fill-in able forms in word. I haven't used the product in years though and I don't know if they still make it. I think the company's name started with a C.
