Copy and rename dir from form submission

1) Project Details:

Hi there!

I am looking for someone to make a small PHP script that I can attach to a form that does the following:

1) Admin enters the clients name into a field and clicks go

2) Script copies an entire directory from the source to the destination (both predefined)

3) The script renames the newly copied directory as the clients name
