Team Ranking Page

I just wanted to see how easy it would be to create something like this:

Once the user submitted the info, it could either email me the results or the results could post to another page to show final team rankings. Please let me know if you have any questions. If this is something that you might like to take on, let me know a price and i'll see if it's in my budget. I have another possible project after this one if I get a good price.
