Hey guys,

I'm not sure if this is the right place to put this but I am in desperate need of a form. The form should take a good developer no more than 10-15 minutes to complete.

I am looking for 4 individual forms (all the same but on different pages). The user fills out 1 form on each of the first 3 pages (textbox). The last page gathers and shows a confirmation page with all inputs made by the user. Once the user hits send, all of the data gets emailed to me.

Ummm...you do know this won't work for all users unless you send the email from server-side code? PHP or ASP or JSP, etc.

Sending email using only HTML/JavaScript only works for those users who have an email client hooked up properly to their browser, and even then only for some browsers.

So maybe the first thing you need to specify is what kind of server-side system you are going to use.

Hmm I've got lots of research to do then. The form doesn't necessarily have to email me I guess (I thought that's what forms did). I'm just looking to see the user's inputs after they push the submit button.

For instance, page 1 has the following text fields:
First Name
Last Name

The next page has radio buttons that asks their gender, etc.

I'm not looking for anything too sophisticated but if I'm going to need to find someone that is fluent in PHP or ASP, then so be it.

Because I asked a question on a separate forum on how long this would take and most replied that it wouldn't take more than 10 minutes. Like I said above, I don't know what or how long it would take to develop. No need to post in the thread about what the "hell" I know and don't know if you're not going to add anything productive.

If you choose PHP as language, please let me know. I'd gladly make your forms and hook them up to a SQL-database.
If you want it to be sent as eMail to you, you need an smtp server (or similar, ain't that much into that stuff).

Why don't you use a web form generator? There are a lot of good ones out there that can help you add and customize the fields that you want in no time. I may recommend 123ContactForm, that integrates well on any page and spares the time of programming a database.
