Triple Drop Down Menu HELP!!

Hey everyone. I need some help with the triple drop down menu (fount at http://javascriptkit.com/script/script2/triplecombo.shtml). I don't want the visitor to be directed to a website once they choose the third drop down menu. I want them to be directed once they click on a button or image that says "go", or "submit". I want this to direct them to the URL specified in the last (third) drop down menu.

Please help me out with this.
