PHP/MySQL Coder - Reaction Page

1) Project Details: Need a Reaction Sheet page created. Drop List to select what File you a sending reaction for, Name field, star rating system, comment box (20 words or less). CMS side where I can update the dropdown list, see who has filled in the form and see an average Star rating for the file.

2) Payment method/ details (Paypal, check? Timeline?): Paypal
