Looking for a coder that works with forms

WE have a situation where i have 4 forms on one page, first we have the text fields,
then we have pay by month, pay annual, subscription. The pay monthly and pay annual needs to be tied in with the text fields and required.

Lastly the subscription I have the code but we need to add additional java for an increase in transaction fee of 0.038% per transaction per month.
