View Full Version : Please Help, Time Crunch - Make a Form

Hi,
I hope someone is up right now and can give me a hand with this. I have a form that I need made. I can make this stuff but I code by hand and it would take some time. If you go to http://tuscaloosaneeds.com/ and click on the Print Out Volunteer Forms link, you'll find a pdf. The first page is a form that I need some help with. If you use some kind of wysiwyg editor and can knock this out quickly, I'd really appreciate it.

I found out today that the volunteer efforts are all being done on paper because most of our emergency response organizations were hit by the tornado that plowed through Tuscaloosa a few days ago. I was asked to come up with something. I'm working on the back end right now but if someone could make that form using just plain ol' html with no style, that'd really help so much.

BTW, This is a volunteer effort and you will be paid the same as I am receiving....feeling that you're doing something good.

Would you want it sent to an email address on submit? If so, I think I can help you out, I'll post the code shortly.

I've attached the code for most of the form. I didn't put in all the different skill options but you can copy the code that I used for the medical part and repeat it for the others. Just make sure that you change the name and id for each of the check boxes. I also didn't include the check boxes in the required information and in the info which will be submitted but there again you can copy from the other fields which are listed and tweak the names and ids.

You'll also have to go and replace the "insert_email_address" text with the correct email addresses and you'll need to specify the action that the form should take.

This definitely should give you a jump start.
