build an app for retrieving static messages from an web hosted MySQL Database. The app will retrieve a message based on a registration code and allow the user to fill out and submit a simple form. this form must then be email to a designated contact for their registration code.

Registration codes are important as 1 user may be connected with multiple codes and will need to select which group they are participating in as well as add and remove groups.
