Custom Form Builder / Router .net

Looking for a developer to build a custom form builder - asp.net 3.5 vb/sql 2008. Similiar to http://www.jotform.com/ that can integrate with our current web application.

The form must be able to be saved/edited within sql 2008 and administrators must have the ability to assign an approval system based on each form created. The system will generate at least 10,000 form submission's monthly.
