Fixing a contact form in PHP / Ajax

I'm building my own website but am not good at the scripts.

How much would any skilled coders here charge to sort out this form:

I took it from here:

But the uploader's no good and needs replacing with the uploader button on the left here:

http://valums.com/files/2009/ajax-upload/demo-jquery.htm

Then the whole form should be made to look like this:

http://web-kreation.com/demos/LightForm

... and then slotted into the centre stage of the website template so it looks roughly like this:
