A Survey that gives you text options on certain questions but on others it gives you a disagree or agree option.

Kind of like this one : http://www.unipark.de/uc/deviantart/ospe.php3?SES=faf003e3b51eee61136a0ffef97544a3&syid=268434&sid=268435&act=start&js=16&flash=1000

With maybe the same type of stuff.

I'm mainly in need of a complete tutorial of some sort from some of you really helpful people here. c;

So please help me. Thank you.

I think your going to have just write it in PHP. On the form use checkboxes, radio buttons and text fields to capture the user data. Shouldn't be too hard to do.

Can you code in PHP?

I can, only very slightly though.

I may need a little bit taught to me by any of the members of here...

The two issues here: you can do this at a rudimentary level with very basic logic. I could outline the logic, but I (and I'm sure others) won't write code for it... that'll be on you.

You can also do this at a complex level, something full featured and effective, and again I don't think anyone will write the code for it.

The thing is, this topic isn't something something so commonly done that they'll likely be many tutorials online. Have you gotten started on the logic pattern for this at all? I'll help ya out, but not if I'm starting you from the ground up.

EDIT: apparently I also fail at googling :p

Did you even try to search google for this?
