There is much information here (http://www.exitfegs.co.uk/Sources.html). Right hand column. Use the search box.
CSS3 makes it easy. This Sources program (http://www.exitfegs.co.uk/Sources.html) uses CSS3 to get rounded corners which will not show in IE yet but degrades well. Firefox and Chrome show it OK.

Have a go and get back to us if you need to. Show us what code you have written or better still a link to the page.
You will probably need to make a background image for the div you show. You will appreciate that as it is a school project, we cannot code it for you. But we can and will help where it is appropriate.

This site looks helpful. (http://www.picment.com/articles/css/funwithforms/)

Frank (http://www.exitfegs.co.uk/)

and this one. (http://www.wittysparks.com/2008/08/24/create-beautiful-form-fields-using-css-and-images/)

Google found these. (http://www.google.co.uk/#hl=en&source=hp&q=css+for+form+box+with+background+image&aq=f&aqi=&aql=&oq=&gs_rfai=&fp=49f4e8b863d17f2e)
