If I wanted to create a form that would send or route someone to a webpage based upon their zip code input, what would be the easiest way to go about this? Is there a simple tool available to do this? I'm thinking of some type of plugin?

A plugin will only work if you are using WordPress or some type of solution that allows plugins. A plugin does not work with a simple HTML form.
I would use PHP code and route the user based upon the zip code they enter. You may need a database containing all of the zip codes if you plan to route the user to many different pages.
If you can provide more details, someone will help point you in the right direction. We need to know what type of software you are using, how many different pages you plan to route a user to and what zip code criteria will determine where a user gets sent.

Use PHP for this. One way you can do it, is create a different webpage per zip code. So when they type in 29901 it goes like this
But it depends on what you want to do with it, what will be on the page, etc.

Do you have maybe a list of 20 or so zipcodes?
And you already know what's supposed to happen when they enter them?
