Welcome here !
This is kind of a strange question, and in fact i don't quite understand what you're trying to do. Compose a fax in an online form and then send it through? Something like that? How strange. But as always, it exist. "Fax over IP"
Think this was the most usefull site (with free services and fax-server info etc etc etc)
http://www.savetz.com/fax/
others
http://go.efax.com/s/r/busn#
(site looks kinda crappy to me)
or
http://www.isendit.net/index.asp?keywords=looksmart
or
http://www.villagefax.net/
(looks somewhat more professional)
IMHO, only "fax to email" is a useful feature. My provider has this as a free service for all accounts. You get a PUM number (= faxnuber) but if someone faxes or sends a voice-message to it, it's converted into a PDF-file or WAV-file. Can't imagen lots of people having a faxmachine but no e-mail ...

I just found this:
www.faxitnice.com
Works great and is nice and simple! No set up fee's or monthly charges. Just pay and go. Its a lot easier to do this in linux then deal with the other faxing options available
