I need a form/script to use in a regular web page or in a pop-up or pop-under where people can include their name and e-mail address and direct that info to my e-mail address or autoresponder. I hope I was clear.

This question has been asked many times- how to submit a form's content to your email address. It's not a question for JavaScript, as the language as purely client side.
