How do I create a Quiz or Test using javascript?

I need to know how to create a quiz or test using javascript or html. EX:

Who is ....?

A. fjkalfjkas
B. fkjad;lsfj
C. fjkaljdfk
D. fjladfkjf

and then have a NEXT button
at the end would add up the results with a finishing grade
