Need a custom Microsoft Word resume template made

Hello,
I designed my resume in Photoshop because I wanted my resume to have a specific look that is different than everyone else. However, I'm not an expert in customizing Word, hence, I'd like to have an expert convert my Photoshop design template (http://i464.photobucket.com/albums/rr2/dosstx/fakeresume.gif) into a Word template that I can edit. I envision it being about 2 pages total.
