Send to Email with Subject Matter

Hi,

I'm not really sure if this a "javascript" question, though it does seem simple enough...er, what's the simplest way to send the results/replies of a feedback form to an email address and have a "subject" matter headlined on the reply (that way i'll know from which feedback form page it came from)??

Don't use spaces in the subject line. Instead use "%20". :D
