Seeking assistance with email form

Would anyone be willing to assist with a flash email form? I need one that is no larger than 190 pixels high. Name, email, and a message (textarea) box are all I need...all required. My host runs php but perl would be fine as well?

Is this a pain?

im not to sure about the limitations of flash but i can help you with a simple php code for emailing.

mail("address","subject","content",'From: your@emailaddress.com');
