Two forms and a database needed

1) Project Details: I am need two forms created based off of the designs below. The Play Log form will need to email the data entered to a person who will then input that data into the other form which the will update the database and out put a table showing player stats.

I'm not 100% sure how this part should work. I am thinking players will need to have an account and login. Once logged in, the form will fill in their name and allow them to select from a drop down which pilot they are updating. They would need to choose who their Wing Commander is (see Wing Commander Log below).

Ace Skills: I have them shown as a drop down, but I think I would rather have that pulled from the database that the Wing Cmnd. updates.

Medals: I have them shown as a drop down, but I think I would rather have that pulled from the database that the Wing Cmnd. updates.

Side/Sorties/Health/Kills/Points: These fields should all be calculated and updated based on the information entered in the Mission Stats section of the form. I'm currently using an Excel spreadsheet to do this, so I have all the formulas need for the math... at least in Excel format. I forgot to add a health field to the sample pic, but it would need to be in there as well.

Mission Stats: This is where players would enter in the results from a game that they played. The Player Name and Pilot Name would be a drop down of only the other players and pilots that have the same Wing Cmnd. They would enter in numeric values in to the remaining fields. This data would update a database table ('m thinking it should add a new line of data for each entry as shown in the pic). That database table keep a running total for the P. Points and Health as well as the Kills and Sortie fields up top.

Mission: This would be a drop down from a list of mission names/numbers that I (as the game organizer) enter into the database.

Sortie: A simple drop down or text filled for them to enter the number of the sortie (each mission has more then one sortie flown for it).

Date: Just the date the mission was flown.

Mission Stats

This is where the Wing Commander would enter in the data he got from a player using the Player Log. I want it so that the Wing Cmnd. has to enter in the official stats so the players can be checked for accuracy. When the Wing Cmnd. submits the data, the data base should add a entry to the table at the bottom of the form/page.

Output Page

I would like a nice table output similar to the HTML one I'm hand coding now... (http://www.gamershavenco.com/index.php?page=wingsofwar). If there is a way to clean it up and make it easier to read, I would really like to do that. Maybe add some ways to filter the display orders?

2) Payment Amount: I've posted this in the Small Projects section, but I think it might be pushing those limits. I would like to keep the cost of this project between $75 and $150 if possible.

3) Payment method/ details (Paypal, check? Timeline?): I can do Paypal or certified funds, which ever works best for both of us. The timeline for this project is NOW or as soon as possible. Funds are set aside already and I'm just waiting on someone to help with the coding part.

Admin Area

I need an Admin area where I can define several things that will effect what options are on the Player Registration page, Wing Log and Player Log forms below.

1. Campaign Year: I need to be able to switch between 1916, 1917, and 1918.

2. Depending on which of those years I select, the following types of aircraft will be available on the Player Registration page:

3. List of every Mission that will be played.

Player Registration Page

This page is where players will enter in their real name, pilot name, side they are playing for (either Entente or Central Powers). Based on what side the player picks and what Campaign Year the Admin has selected, the player will be able to select the type of aircraft they will fly via a drop down menu. They will also need to select who their Wing Commander is from the registered Wing Commanders.

I will be running this site/system in conjunction with a vBulletin forum, so if it could use the same username/password/cookies that would be even better.

Upon registering or logging in the player will be taken to the Player Log page.

Player Log

The above image is of a new Player Log Page before any data has been entered by the player. I will break down each section of the page/form...


Player Name: Filled in when logged in.

Pilot Name: Players can have more then one Pilot, so this would be a drop down list of all his Pilots.
Side: Filled in once they select the Pilot Name. Based on choice at registration.

Wing Commander: Filled in once they select the Pilot Name. Based on choice at registration. This is the person that will receive the data from this form when the player hits the Submit button.

Sorties: A running total of games played.

Kills: A running total of how many kills they have.

Health: A running total of the pilot's health. Starts at 25 and goes down as pilot suffers damage as entered in the Mission Stat section.

Points: A running total of the pilot's points.

Mission Stats

Date: Date entered in by player for the date a game was played

Opponent's Name: The player they played against. This should be a drop down menu based on every player that selected the same Wing Commander at signup as the one at the top of this page.

Pilot's Name: A drop down list of every pilot the Opponent has.

Kills: A field where the player enters the number of aircraft that he shot down. A valid value would be 0-9.

Chased Off: A field where the player enters the number of aircraft that he chased off the table. A valid value would be 0-9.

Recon: A field where the player enters the number of ground targets that he successfully observed. A valid value would be 0-9.

Strafing: A field where the player enters the number of ground targets that he successfully destroyed by strafing. A valid value would be 0-9.

Bombing: A field where the player enters the number of ground targets that he successfully destroyed by bombing. A valid value would be 0-9.

Shot Down: A field where the player enters the number of times he was shot down. A valid value would be 0-2.

POW: A check box that the player selects if his pilot was captured. If this box is checked when the form is submitted, the form/database entry for this PLAYER / PILOT combo should be locked and no longer editable by the player.

KIA: A check box that the player selects if his pilot was killed. If this box is checked when the form is submitted, the form/database entry for this PLAYER / PILOT combo should be locked and no longer editable by the player.

The image below is what the Player Log would look like after a player has entered in some data. Each game he enters would add another row of data to the table at the bottom of the page. As this data gets added, it updates the fields at the top of the page labeled Sorties, Kills, Health and Points. Each time a player submits data, it should be sent to the Wing Commander for review and entry into the Wing Log. This step is to validate the data entered by the players. If you can think of a better way to do this, let me know!

The blue fields would be added and filled in based off of data entered on the Wing Log.

Wing Commander Registration / Login

This page is only needed to give access to the Wing Log page / form below. Wing Commanders can also be Players. Again, having the username and password / cookies working with the vBulletin forum would be nice.

The image above is the Wing Log before any data has been entered. The Wing Commander will take all the data sent to him from the Player Log and enter it into the Mission Stats section of this form. I will break down all of the fields on this page / form...

Date: The date that a game was played.

Mission: The Mission Number and Name that was played. This needs to be added to the database by the Admin in the beginning.

Sortie: The number of games that were played for that Mission. A valid value would be 1-9.

The image below shows the Wing Log after some data has been entered. A new row would be added at the bottom of the page each time the Wing Commander submits data from this form.
