1) Project Details: I am need two forms created based off of the designs below. The Play Log form will need to email the data entered to a person who will then input that data into the other form which the will update the database and out put a table showing player stats.

I'm not 100% sure how this part should work. I am thinking players will need to have an account and login. Once logged in, the form will fill in their name and allow them to select from a drop down which pilot they are updating. They would need to choose who their Wing Commander is (see Wing Commander Log below).
Ace Skills: I have them shown as a drop down, but I think I would rather have that pulled from the database that the Wing Cmnd. updates.
Medals: I have them shown as a drop down, but I think I would rather have that pulled from the database that the Wing Cmnd. updates.
Side/Sorties/Health/Kills/Points: These fields should all be calculated and updated based on the information entered in the Mission Stats section of the form. I'm currently using an Excel spreadsheet to do this, so I have all the formulas need for the math... at least in Excel format. I forgot to add a health field to the sample pic, but it would need to be in there as well.
Mission Stats: This is where players would enter in the results from a game that they played. The Player Name and Pilot Name would be a drop down of only the other players and pilots that have the same Wing Cmnd. They would enter in numeric values in to the remaining fields. This data would update a database table ('m thinking it should add a new line of data for each entry as shown in the pic). That database table keep a running total for the P. Points and Health as well as the Kills and Sortie fields up top.

Mission: This would be a drop down from a list of mission names/numbers that I (as the game organizer) enter into the database.
Sortie: A simple drop down or text filled for them to enter the number of the sortie (each mission has more then one sortie flown for it).
Date: Just the date the mission was flown.
Mission Stats
This is where the Wing Commander would enter in the data he got from a player using the Player Log. I want it so that the Wing Cmnd. has to enter in the official stats so the players can be checked for accuracy. When the Wing Cmnd. submits the data, the data base should add a entry to the table at the bottom of the form/page.
Output Page
I would like a nice table output similar to the HTML one I'm hand coding now... (http://www.gamershavenco.com/index.php?page=wingsofwar). If there is a way to clean it up and make it easier to read, I would really like to do that. Maybe add some ways to filter the display orders?
2) Payment Amount: I've posted this in the Small Projects section, but I think it might be pushing those limits. I would like to keep the cost of this project between $75 and $150 if possible.
3) Payment method/ details (Paypal, check? Timeline?): I can do Paypal or certified funds, which ever works best for both of us. The timeline for this project is NOW or as soon as possible. Funds are set aside already and I'm just waiting on someone to help with the coding part.
