Simple form attachment

Hey Guys,

What I would like to do is just have a very simple form where the user can attach an image to the form and it is in an attachment when I get the email.

I have built the form processor, and everything sends and works fine. I thought I would just get away with a File input box lmao but it turns out it doesn't work that way.

I would appreciate any help.

I'm actually a bit embarrased to show my coding lmao..I am extremely messy and my php sucks. I have attached activity1.php which is the page the form is on and also petsend.php which is the processor.
