Clicking link to php file forces user to download file?

Hi all,

I inherited a website that has a third party PHP contact form. I am not a PHP person, so I am not sure why when I click the link to the form, it keeps asking me to download the php file. I have php correctly installed on the server - but perhaps not the right version? (I installed PHP5). Please check it out at:

Click "accept" at the bottom. I am thinking this is just a route or config simple issue...

The link on the html file was pointing to the wrong place. Thank you for looking though.
