How do i create this:

i got this textfield

fill in your name so we
can check your account</p>
Name:&nbsp;<input maxlength="30">
<br>
<input value="Sumbit" type="submit"><br>


and if they press sumbit how can i make it go to index.php?name=(the filled in name) and the file where they need to go is called: stats.php and the variable is $sendedname please can someone create it for me :p

Just by using a simple form (http://www.htmldog.com/guides/htmlbeginner/forms/) having method="get"

and how do i create the thingy that if you do
index.php?name=lol that he shows the 'stats.php' page?
