Create a text box (input box) in second drop down menu

Hi ,

I am required to convert a one menu of a double drop down menu to text box or input box.

i.e. If One menu of double drop down box is "your name" and in second drop down box will convert to text box so use can actually add name in the text box.

How can I do this using java script as I am very new Bee.
