PHP Script for User feedback.

Hey there,
I'm in need of a PHP Script that can be used for some clients at my school.
I have a set of questions, about 10 of them, they will all be answered with either 'yes' or 'no', each question answered 'yes' should hold %10 and 'no' with %0. When a user is through responding to the questions, php can now say all questions amount to %60 if the user responded with 'yes' to 6 questions and such.

Does anyone know of a script I can use to accomplish this, or something similar I can modify to accomplish something of this nature.
All help and responses would be greatly appreciated.

I clicked the 'Thank user for this post' button yesterday, but couldn't reply, I was too lazy to start punching my phone's keypad :P

While they were a good number of resutlts, I couldn't find anything easy to setup and something that'd fit the purpose stated above, thus, I still require assistance with this. Has anyone here used a script and found it good enough to recommend?

Surely you're not implying you tried all 41 scripts found on the Hot Scripts website? (http://www.hotscripts.com/PHP/Scripts_and_Programs/Tests_and_Quizzes/index.html) Are your requirements so much different that it isn't out there ready for you to use?
