Basic PHP/MySQL Form Script Needed

1) Project Details: (be as specific as possible):
Script will have 3 steps
1) A form in which user submits there Name, Comment, and Email. They can choose to have there name and comment private or public.
2) When click "submit" on the form from step 1. there info is stored in a database and they are given a unique ID. They will be sent to "Page 2"
3) On page 2 I will display a thank you message along with a link to one of my sponsors. This link must have appended to it the ID of the user who just submitted the form on page 1 (www.somesite.com?user=ID).

2) Payment Amount:
I think this script should not cost more then $100. I could do it myself but I would rather have someone decent do it that will have all the security aspects covered. If doing so will cost more then $100 then that's fine. Price is an issue to some extent but I would rather pay more and get good work.

3) Payment method/ details (Paypal, check? Timeline?):

The link on page 2 will be static. Well, The only thing that will change is the User ID from page 1 that is added to it.
