checkbox not working with cfm form

I keep getting errors with the checkboxes on this form: http://www.gatequest.net/request.hosting.cfm

Just fill in the two email fields at the bottom (fake email is fine) to see the error.

Remember if the box is not checked, the value is not passed from the form to the processing page.

I didn't bother filling out your form, it looked like you took the check marks off for raido buttons.

are you still getting errors?
