File upload/transfer services - need advice

Hi -

I am developing a personal (non-business) site from which users will be submitting/uploading files and text to me.

I could certainly use email as a quick-and-dirty solution, but I would prefer to show the user an html form, have them fill in a few selection boxes, enter text, and attach up to maybe 5 files.

I need to use a 3rd-party file upload/transfer service to enable this...

1) I am looking for a free or low-cost service. I want to be able to imbed html so it is right on my site. If it contains minor branding, that's ok. It needs to have decent capacity, because the files will be jpg photos. I understand that these services take the uploaded files and store them on their server, I can download from there.

2) I would be just as happy with the following: A service that would look exactly the same to the user - an html form imbedded in a web page enabling the uploading of various info, plus up to 5 files. However, I would be very happy if the service simply emailed me the uploaded files - no need for them to save the files at all. Do such services exist?

Would be interested in any helpful advice...
