CGI Form Mail Troubleshooting

Hi, in my first attempt to upload a form online I used Matt's free CGI Perl script (I'm on a mac in Dreamweaver). I cannot figure out what I'm doing wrong, is there a way to troubleshoot this? Can someone recommend a good script to use for the non-coding designer I am?

Matt's script is poorly written and is not secure. A better option would be to use the FormMail script from the nms project.
