Looking for programmer to create a form system for a client

Hello,
I am looking for a programmer to create a form system for a client's screenprint web site.

In general this form takes design information and emails it to site owner. User also receives an email. The is NOT an e-commerce system.

The basic idea is for users of the site to be able to choose a design, enter their design information, then choose ink colors and make garment selections. When choosing garments users should be able to enter number of shirts, shirt color, and sizes to see price total. Information is to be emailed to site owner and a copy to the user.

Site owner plans on having 30 to 50 designs on the site and will want to be able add more in the future.

The deadline for project to be fully completed is 8/1/08.

PHP preferred language.

Project may get extended to include taking concepts of the home page plus other pages (5 to 6 pages) and turning them into CSS/HTML.

If i understand correctly this is a rather small application, the user should be able to select some design enter some information and this will be emailed to someone. Designs can be added/deleted etc.

Assuming this, i can probably provide a working prototype in less than a week, and have it completed in less than two. Those are highly conservative estimates, in reality i'd say it could all be done in a week but with august being your deadline best to do everything right and get it working in the way your client would like.

I would charge $350 for this, i would expect $150 when i can demonstrate a working prototype, $100 when this is refined and working as your client would like and a final $100 when the application is installed on your clients server and working.
