I am looking for someone to write a java script for my website.

Basic entry prompt

Something like "Load File" from website visitors computer

"Enter Email address" "Confirm Email address"

"Enter Name"

All this info, file, email address and name to be captured and sent do me. By email but any automatic method will work,


Let me know your thoughts, examples, and cost.

Would you need the file to be sent to you as well? If this is the case then javascript can't do this.
