when your first creating a website using php, do you start working on the design first or the php files?

I start with the main concept of the site.
If it will require a lot of CMS, or perhaps a blog-like site,
that will determine if I'm going to use a Joomla system,
or perhaps WordPress. Knowing that really changes things
because the site layout will be a template based on those.
If it will be a simpler site, I ask the client to look at various
CSS templates to get an idea of what they are thinking about.
If I know what the "look" is going to be, I don't need to focus
much more on design ... the graphics can come later.
Always look at the "large picture" to determine ahead of time
how much PHP (and MySQL) will be used in the site.

I am in the process of doing my first php / mySQL website and as I am building from the gound up I can not use CMS so here is the process that I am following - in this example, it is how I did the menu, but everything is done the same.
Worked out a very basic layout
Worked out what kind of menu I wanted - ie: drop down
added a php "hard code" file for the menu
created the menu tables, then the add, edit delete php files for the menu
changed the menu php "hard code" to dynamic code from the table
I also use alot of include files so I can move things around easily.
