Need help creating form that generates a PDF.

So here is that I would like to be able to do.

We have an website site for all of our brokers. Our brokers constantly ask us in Marketing/Sales for proposals. These proposals vary sometimes in the pages that are included and pricing.

So I would like to create an online form that they can use to choose what they would like contained in their proposal. At the end of the form they would submit it and an un-editable PDF would be generated for them that contains whatever pages they selected along with the appropriate pricing.

Any ideas on how to go about this? All I need is a shove in the right direction, because all of my web searches are turning up nothing.

Collecting the data (via the form) is trivial if you know what you want collected, and it wouldn't be complicated creating the pdf with PHP's PDF functions (http://www.php.net/pdf). So that's how I'd do it, with PHP and those PDF functions. If you need help ask in the PHP forum (http://codingforums.com/forumdisplay.php?f=6).

starbucks_adct....I would recommend you check out http://www.fpdf.org they will have 100% of what you need.

I researched both PHP's PDF functions and fpdf.org and in my opinion fpdf.org is much better. They have lots of sample codes, tutorials and examples on the http://www.fpdf.org site.
