Making a email form from an image???

I`m trying to make an email form from an image but i`m not having much luck.

Is there anyone who could help me out as i`m new to site design and i`ve no idea how to do it.

Hmmm, might I suggest a begninner's HTML book? Unless you find a very generous individual, you will have to learn how or pay someone to do it.

People like myself run a business doing stuff like this for those that don't know how. And while I'm very affordable, a basic HTML book would be cheaper, but would take longer. There's really too much to explain to cover it all here.
