Dreamweaver Insert Record Broken

Hey,
Could anyone with Dreamweaver CS3 send me InsertRecord.htm and InsertRecord.js they are stored in C:\Program Files\Adobe\Adobe Dreamweaver CS3\configuration\ServerBehaviors\PHP_MySQL\.

i do have them jst one of the corrupted, a friend sent to me and it works now :)
