I have an HTML form with about 5 or 6 fields that I need sent to a comma-delineated .txt file, preferably via PHP. I want one file continuously augmented when new viewers submit information, not a new .txt file every time.

Only caveat here is that I need there to be an if/then clause in the code (excuse my syntax, I'm not a PHP coder), such that users must check a radio box with a licensing agreement. If they try to submit the forms without agreeing to the license, an error message needs to appear.

If possible, a confirmation email would be sent upon submission.
