TOAD has a version for mssql. You can use it for free for a while. Otherwise you have to fall back to asp.net enterprise manager (search on google for it). It is no longer maintained i believe, but i could be wrong.
Don't know of other good ones

I've used EMS SQL Manager Lite (http://www.snapfiles.com/get/emsmssqllite.html) for quite a while now. My biggest gripe about it is that it's nagware, prompting you to upgrade to the latest version on launch, but never letting you do so unless you pay. So whatever version you install, that's the version you're stuck with. Having said that, it will let you create databases and tables, create users, change passwords, modify data, and other such stuff. It is somewhat crippled, though, in that you can't create stored procedure calls or other more advanced actions unless you buy the full version. But the Lite version is not time limited.
