Hi, i found this forum by searching 1 script in internet.
I am leader of Clan in Counter Strike Source (Online FPS game) and i have a web ( http://ev0.clans.net ). On that web i have a page with recruitment (Try out), and i was needed a script that can collect Peoples Answers that they wrote in that form and script could send those answers on a specific email.

For example :
Real Name (Box to be filled)
Location (Box to be filled)
Sex (Choice (*)Male ( )Female)
And etc.

I was looking for that script WHOLE my life because i need it so much...
I really appreciate your help.

You can search Google for: php form emailer

There are many examples and tutorials to choose from.

One thing to keep in mind .... you'll get some spammers that will latch
onto your form and you'll get hammered with spam emails. Not much
you can do about that unless you only allow "registered members" to
fill out your questionnaire.

If you have no PHP scripting skills, you can hire someone to do it for you.

It would be relatively simple ... depending on security requirements.

.shtml refers to SSI (Server Side Includes)...

That is somewhat over now, since everyone uses PHP instead.
Almost all webhosts now have PHP installed and enabled.

So you'll be looking for a PHP script.

It's not incredibly difficult to create a PHP script yourself. Thats part of the beauty of PHP and why it's so popular - its a simple language, thats well structured.

It wouldn't be out of your realm to create a nice simple script which takes all the users input and places it into a mySQL database.
