I need a MySql database for my website. I have been unsuccessful in any attempt to log in to MySql via telnet and my e-mail messages to support have gone unanswered. How do I get a database established?

Usually web hosts have a control panel that gives you some kind of tool to manage your databases. Have you checked to see if you have such tool? If you don't then you may want to look into PHPMyAdmin (http://phpmyadmin.net).

do you have mysql on your host? did you create a database, and a table and connect to the database correctly?

there a numerous tutorials out there check them out,
if you need a host with mysql freehostia.com offers a good service, although lately they have been down a bit. :( hopefully they sort their problems out though
