Creating a nice little form!

Ok I need the following :

A form with name , e-mail, subject and a text area. Also i want the option to upload a file with the form.

Obviously a message would be displayed after the message has been submitted - with the whole form being e-mailed to a pre decided address.

What will make this easy for me!?
