Someone mind helping out?

I'm having a little trouble guys. I am doing a website for a local company and am having one heck of a time trying to figure out how to do their contact form. If someone would like to write it for me and help me out i'd be willing to pay. I basicly need a modified contact sheet that goes straight to email similar to this design here:
