Good DB Modeling Tool

I do realize that this questions isn't about a query, but here it goes.

I'm wondering which tool you guys recommend for creating a database model. This will be used for MySQL and possibly SQL Server 2000/2005. Also, I'd like to have the capability to reverse engineer my diagram to create the appropriate SQL code. Price isn't much of an issue if such software must be purchased.

I Googled around a bit but couldn't find exactly what I was looking for.

As far as MySql concerns there is the DBDesigner 4

This is great and it is being updated for MySql5 and integrated into 'MySql Tools' as 'MySQL Workbench'. It is still a bit buggy.

Furthermore there is a plenty of commercial software, like ErWin, ERstudio from Embarcadero (a pretty nice tool), TOAD Modeller but they cost. You can also look at some UML tools which include db design possibility.
