How to insert multiple form elements to a table

Hi,

I am currently in the process of trying to get the form elements that I post to another page. The problem is that I have created checkboxes and textboxes dynamically w/AD (over 400 of them). The user checks the boxes that they wish to add date values for. Any ideas on how I might go about inserting a record for each of the 400+ form values?
